Long-term care issues have high prominence among state officials in Pennsylvania as a result of its large elderly population and concern about the impact of long-term care costs on the state''s budget. Its population aged 65 and older is 15.6% of its total population, ranking second only to Florida. By 2025, 21% of its population will be 65 and older. Pennsylvania is federal and state Medicaid spending for long-term care in FY2001 was $5.1 billion -- almost half of all Medicaid spending. Spending for nursing homes was more than one-third of Medicaid spending. While spending for home and community-based services has increased dramatically in recent years, these services represented less than one of every five dollars spent on long-term care in FY2001. Over the last two decades, Pennsylvania has documented issues it has confronted in providing long-term services. Among these issues are: an imbalance in financing favoring institutional care, rather than home and community-based care (which most people prefer); fragmentation in the management and delivery of services; difficult access to services, especially for low and moderate income persons who do not qualify for Medicaid; and disparities in service availability across the state and populations in need of care. According to state officials, Pennsylvania''s guiding principles in long-term care are to: control surplus growth of nursing home beds; support consumer choice; encourage expansion of home and community-based services; fund services rather than capital construction; and assure quality of care.

The first wave of the 78 million member baby boom generation is now reaching retirement age. The number of people age 62, the earliest age of eligibility for Social Security retired worker benefits, is expected to be 21 percent higher in 2009 than in 2008. In addition, by 2030, the number of workers supporting each retiree is projected to be 2.2, down from 3.3 in 2006. This demographic shift poses challenges to the economy, federal tax revenues, the nation''s old-age programs, and individuals'' financial security in retirement. For those who are able to work longer, later retirement can strengthen the economy and also retiree incomes by postponing the time at which people will start drawing retirement benefits rather than working. A wide range of factors including the features of employers'' benefit plans, personal finances, social norms, health, and individual attitudes influence workers'' decisions about when to retire. Federal policies may also play a role: these include Social Security, Medicare, and tax policies related to certain private retiree health and defined benefit (DB) and defined contribution (DC) pension plans.1 Identifying both the incentives posed by these policies and the extent to which workers respond to them can help to inform policy makers as they consider ways to address the demographic challenges facing the nation. To determine the extent to which federal policiesdirectly and indirectly-pose incentives and are influencing individuals'' decisions about the age at which they retire, the authors have pursued the following questions: (1) What incentives do federal policies provide about when to retire? (2) What are the recent retirement patterns, and is there evidence that recent changes in Social Security requirements have resulted in later retirements? (3) Is there evidence that tax-favored private retiree health insurance and pension benefits have influenced when people retire? This is a revised and excerpted version.

After earlier criticism from human rights organisations and many foreign governments regarding the determination that the Geneva Conventions of 1949 do not apply to the detainees held in Cuba, President Bush shifted position with an announcement that Taliban fighters are covered by the 1949 Geneva Conventions, while al Qaeda fighters are not. Taliban fighters are not to be treated as prisoners of war (POW), however, because they reportedly fail to meet international standards as lawful combatants The decision is not likely to affect the treatment of any of the detainees held at the US. The Geneva Conventions of 1949 create a comprehensive legal regime for the treatment of detainees in an armed conflict. These ''unprivileged'' or ''unlawful combatants'' may be punished for acts of violence for which legitimate combatants could not be punished. Some have argued that there is implied in the Geneva Conventions a third category comprised of combatants from militias who do not qualify for POW status but also fall outside of the protection for civilians. These combatants may be lawful in the sense that they do not incur criminal liability for engaging in otherwise lawful combat, but they would not be entitled to privileges as POWs or protected civilians. The status of the detainees may affect their treatment in several ways. This book explores the dilemma that the US government faces as far as the classification of its prisoners that they deem to not fall under the POW status and the treatment and rights that they are entitled to.
